Indication

Tachycardie
Ralentissement FA rapide sans défaillance HD (Objectif FC < 110/min)
Dissection aortique (Objectifs : PA<120 mmHg et FC entre 60 et 80/min)

Posologie / Préparation

Bolus de 0,5 mg/kg IVL 1 min puis 0,05 à 0,3 mg/kg/min IVSE par pallier de 5 min.
Si échec : on refait 1 bolus et on augmente le PSE au pallier suivant.
Si efficace : on poursuit au dernier pallier utilisé.

Contre-indication

Bradycardie sévère ou hypotension sévère
BAV
Choc cardiogénique ou insuffisance cardiaque congestive
Crise d’asthme
Acidose métabolique
Phéochromocytome non traité

Précaution d’emploi

Surveillance scopée
Association avec d’autres antiarythmiques (notamment prise de vérapamil dans les 48h)
